This is a custom page that has been added to the teacher/student interface. This page is a two-step process that allows a data point (piece of flex grid) to be added to a student record (customer).

Step one of the process asks you to select the age of the student, the "core" subject, and the sub section. Once submitted, you will be shown your previous selections and prompted for a couple more choices. The two-step process allows the page to only show relative filtered options. Without the second step, the page would show way too many options to choose from.

Step two asks for an objective, a rating score, a date, and any optional notes. There is also an alternate rating field in case the normal rating score does not fit the current selection.

Once the page has been submitted (end of step two), you will be rerouted to the student or customer log page to view the new details and/or data points.
